Sawyer here from Applied Fitness Solutions.
If you’re reading this, it likely means we just got off the phone, and that you’ve made the exciting decision to work with us.
First off, thank you for trusting us in this process. We recognize that the fitness journey can be quite a personal experience and we do not take this trust lightly. We are committed to supporting you on your journey to a more fulfilled life.
Before we can get started, we need to collect some information in preparation for your New Client Orientation (NCO), which should have just been scheduled during our call.
Please fill out the form below so that we can complete your AFS profile prior to our meeting. Additionally, please take a moment to review our membership agreement below which contains important information on the specifics of your membership.
Lastly, we ask that you would please be sure to download the AFS Connect App from Google Play or Apple App Store. During your orientation meeting we will set your app account up for access and explain how it may be used in the coaching process. For now, if you can simply download it that would be helpful during your orientation.
